Health declaration & Screening Information:
We will obtain information about your potential exposure to COVID through your vaccination certificate. As part of this, we may obtain other information related to the vaccination status.
Access to our services is currently limited to individuals who has received a COVID-19 vaccination. If you work with us, you will be required to provide vaccination certificate that you have received a COVID-19 vaccination, an acceptable vaccination certificate. This information is obtained when you present your vaccination certificate. We will keep a record of your vaccination status and certificate to facilitate future visits. Prioritize the safety of our staff and members, identify potential COVID-19 exposure, regulate access to our services including the Standard Operating Procedure (SOP).
How We May Share Screening Information:
While we generally will not share your Screening Information without your consent, we reserve the right to disclose Screening Information, and other related information, to the extent required to do so by governmental or regulatory authorities or as otherwise necessary. If we are notified that you tested positive for COVID-19 within 15 days after your last visit in our services, we reserve the right to disclose to our staff and members that an individual who tested positive, participated in activity and practices; we may disclose your name when legally required to do so, we may disclose information such as the date and time of your visit, your gender, and classes you have entered . We share this information to help ensure and remain safe for our staff and other members. We may also share your Screening Information and other related information with service providers, advisors or consultants who are performing services on our behalf and for other reasons in accordance with applicable law, as further described.
We might disclose your information to competent law enforcement bodies, regulatory authorities, government agencies, courts, or other third-party entities if we believe it is necessary to comply with applicable laws and regulations, to protect and assert our legal rights, or to safeguard your vital interests or the vital interests of others.
How We Maintain Screening Information:
The screening information, in the Privacy Policy, will be used to review and verify your vaccination certificate and any other relevant information. We may record and indicate your health declaration, vaccination status, and other related information. Aside from the mentioned, any additional screening information that we keep will be stored and as required by law.
